Madrid, 25 de octubre de 2006. “El médico debe adaptarse a la situación actual sin renunciar a la búsqueda de la excelencia, su santo y seña a lo largo de la historia”, ha destacado el Profesor Diego Gracia, presidente de la Fundación de Ciencias de la Salud, en el VIII Ateneo de Bioética, celebrado esta mañana en Madrid y organizado por la Fundación de Ciencias de la Salud y el Instituto de Prevención, Salud y Medio Ambiente de la Fundación MAPFRE, cuyo objetivo es la promoción de la investigación, la docencia y la divulgación en salud.

El acto ha intentado dilucidar el futuro de la profesión médica, respondiendo a preguntas tales como ¿salen los médicos postgraduados con el bagaje humanístico y técnico suficiente? o ¿cuál es el papel de la compasión, la empatía, el respeto y la confianza en un sistema sanitario?

El Profesor Gracia se ha referido a la gestión como el principal obstáculo con el que suele encontrarse el médico a la hora de ejercer su profesión: “No es fácil cumplir con el doble objetivo de buscar el bien del paciente y contener los costes sanitarios”, ha explicado. A esto se une el hecho de que “la población es cada vez más exigente con el sistema sanitario”.

Bajo el título “El nuevo profesionalismo: el médico que deseamos para el siglo XXI”, el VIII Ateneo de Bioética ha puesto de manifiesto el importante cambio experimentado por la profesión médica en las últimas tres décadas. Además de la desaparición de la paternalista figura del médico tradicional, “la conversión de este profesional en un asalariado suele colocarle en situaciones muy comprometidas”, como las provocadas por la necesidad de ahorrar recursos.

El debate se ha realizado en dos partes. En la primera se ha escenificado una representación teatral de 50 minutos de duración que resucita a tres grandes filósofos del pasado, que representan la ética utilitarista (John Stuart Mill), la maduración ética (Lawrence Kohlberg), y la voluntad y libertad del hombre autónomo (Friederich Nietzsche).

Desde estas tres ópticas, “se analizan los retos de una sociedad consumista para la que la enfermedad es cada vez menos aceptable, llegando a ver la muerte no como nuestra realidad más ineludible, sino como una mera posibilidad, remota y abominable”, ha dicho el Dr. Francesc Borrell, médico de familia y coautor de la pieza teatral.

“Un médico optimista, dotado de compasión y empatía, y con una buena preparación técnica, gusta de inmediato y genera confianza”, ha resaltado. En la jornada se han señalado como principales obstáculos de su quehacer diario la falta de tiempo y la sobrecarga de trabajo. “Cuando una institución trata mal a sus empleados, éstos hacen lo mismo con la población”.

En respuesta a una “creciente medicalización” de la vida, también se han apuntado tres retos para la profesión médica del siglo XXI: “no permitir que sólo las instituciones dicten las reglas del juego, definir con valentía el perfil del médico del futuro, y perseverar en que los cuidados de salud y las innovaciones alcancen a todas las clases sociales”, ha enumerado.

Según una encuesta realizada por el Instituto británico para el Estudio del Sector de la Alimentación, un 10% de los adolescentes y un 5% de los adultos no tendría ningún inconveniente en implantarse un chip en su organismo con el fin de identificarse, pagar, etc.

Como todo, existen aspectos positivos y no tan positivos, por un lado se evitarían fraudes y robos como los que se asocian a las tarjetas de crédito. También se evitarían llevar encima documentos como el D.N.I, tarjetas de crédito, etc., evitando robos. Se podría acceder al historial médico de la persona en cuestión en el caso de que fuera necesario, en fin, muchas serían las ventajas.

Los aspectos más negativos, un control absoluto sobre la identidad, el lugar donde nos encontramos e incluso lo que hacemos, violando nuestra intimidad. La verdad es que es algo complicado y hay que añadir que quizás se encontrarían nuevas técnicas y los crímenes proliferarían en busca del ansiado chip para suplantar una personalidad o para realizar algún tipo de estafa.

La práctica de incorporar un chip es evidente en España, en la ciudad de Barcelona, la zona Vip del Baja Beach Club los utiliza, sus clientes tienen implantado un chip que les permite pagar sin necesidad de llevar dinero o alguna tarjeta de crédito. Existen además noticias sobre la implantación de chips en los empleados de determinadas empresas con el fin de tener un control más exhaustivo sobre ellos.

La verdad es que no nos agrada la idea, quizás si el sistema garantizara una total seguridad, una intimidad absoluta y un respeto por los derechos que tenemos, lo aceptaríamos.

El término esquizofrenia es inválido y no debe usarse, dicen expertos en el Día Mundial de la Salud Mental.

La palabra esquizofrenia cataloga incorrectamente a una serie de síntomas.

Varios reconocidos expertos en psiquiatría dicen que esta categoría agrupa incorrectamente a una variedad de síntomas.

Además, agregan, promueve el tratamiento basado en medicamentos y no en la intervención psicológica.

Según los expertos, el nombre también pone estigmas en los enfermos como seres violentos, peligrosos e intratables.

"Por lo general la gente le da muchos significados distintos a la palabra esquizofrénico" dijo a BBC Ciencia el doctor Miguel Ángel Sabogal, presidente de la Asociación Colombiana de Salud Mental y quien también está abogando para cambiar el nombre de la esquizofrenia.

"Y en su mayoría esos significados están asociados con la locura", agrega el experto.

Complejidad

La esquizofrenia representa un complejo trastorno se salud mental.

Los síntomas pueden variar de persona a persona, pero incluyen delirios, alucinaciones y percepciones falsas de la realidad.

Un nombre que se ha propuesto es "síndrome dopaminérgico".

Se calcula que una de cada 100 personas desarrollará esquizofrenia en algún momento de su vida.

Ahora los expertos están pidiendo que el término sea abolido.

"No dudamos que hay gente que tiene experiencias angustiosas como escuchar voces o tener temores paranoicos", dice el profesor Richard Bentall, profesor de psicología clínica experimental de la Universidad de Manchester.

"Pero el concepto de esquizofrenia no tiene ningún valor científico, ya que agrupa a toda una serie de diversos problemas bajo una misma etiqueta", agrega.

Por eso, dice el psiquiatra colombiano Miguel Ángel Sabogal, es importante "darle un nombre que no dé lugar a diferentes especulaciones sino que refleje un solo significado real".

"En sus inicios --explica el doctor Sabogal-- el trastorno se denominó demencia precoz, asemejando la demencia senil".

"Después apareció la palabra esquizofrenia, que significa "mente dividida --agrega el psiquiatra-- pero hacía más referencia al proceso psicológico que al proceso neurológico".

"Pero desde mediados del siglo XX hasta ahora se ha tenido amplia evidencia de que esta es una enfermedad en la que hay una seria alteración biológica y se aparta mucho del concepto original que se dio al término", afirma.

Tratamientos

El término esquizofrenia, dicen los expertos, promueve un uso extenso de medicamentos antipsicóticos como primera línea de tratamiento, en lugar de ofrecer al paciente ayuda psicológica.

Aunque los medicamentos son útiles en algunos pacientes, a menudo se les dan dosis extremadamente altas con peligrosos efectos secundarios.

Los expertos afirman que los pacientes deben tratarse con base en sus síntomas individuales, de lo contrario, se corre el riesgo de ofrecer tratamientos totalmente inapropiados.

Es por eso que se han propuesto nuevos términos que describen mejor a este trastorno.

"Yo he propuesto el nombre de síndrome dopaminérgico", dice Miguel Ángel Sabogal.

Este nombre, dice, refleja con más precisión lo que está sucediendo biológicamente en el cerebro cuando alguien sufre psicosis.

Y lo que sucede es una alteración en los niveles de dopamina --un compuesto químico cerebral-- que produce la aparición de los síntomas como alucinaciones y delirios.

Comer nueces al final de una comida podría reducir el riesgo que los alimentos grasos causan a las arterias, indica una investigación.

Las nueces contienen compuestos que ayudan a la flexibilidad de las arterias.

Se cree que las nueces son ricas en compuestos que reducen el endurecimiento de las arterias y las mantienen flexibles para permitir un óptimo flujo de sangre.

Esa es la conclusión de un estudio llevado a cabo por el Hospital Clínico de Barcelona.

"Logramos obtener una prueba más de que las nueces son buenas para la salud cardiovascular", dijo a BBC Ciencia el doctor Emilio Ros, quien dirigió la investigación que aparece publicada en la Revista del Colegio Estadounidense de Cardiología.

Una comida

Los investigadores reunieron a 24 adultos; la mitad con niveles normales de colesterol, y la otra mitad con niveles moderadamente altos.

El objetivo era conocer el efecto de las nueces en las arterias tras una sola comida.

Se sabe que cuando se ingieren alimentos ricos en grasas saturadas -las que son por lo general de origen animal- se produce una inflamación y oxidación en las arterias.

Fumar conduce al endurecimiento de las arterias y mayor riesgo de infartos.

Con el paso del tiempo, se cree que este efecto causa un endurecimiento de las arterias y un aumento en el riesgo de enfermedades cardiovasculares.

Para el estudio, cada uno de los voluntarios consumió dos comidas cargadas de grasas saturadas, de chorizo, queso extra graso, yogures grasos, etc, separadas por una semana.

En una de las comidas los investigadores añadieron cinco cucharaditas de aceite de oliva.

En la otra comida, añadieron ocho nueces descascaradas.

"Lo que vimos fue que con los bocadillos a los que se había añadido aceite de oliva seguía deteriorándose la función de relajación de las arterias", afirma el doctor Ros.

"Pero al añadir nueces -agrega- no ocurría así, lo que significa que con las nueces se prevenía el efecto adverso de la grasa animal sobre las arterias".

Cuidado

Las arterias que son elásticas son capaces de expandirse cuando es necesario que un mayor flujo sanguíneo llegue a un órgano.

Los factores como fumar, tener altos niveles de colesterol, la obesidad y la hipertensión afectan la flexibilidad de las arterias.

Según el doctor Ros, las comidas ricas en grasas saturadas inhiben la fabricación de óxido nítrico en la capa interior de las arterias, el endotelio.

El óxido nítrico es un compuesto químico necesario para mantener la dilatación de las arterias.

"Es uno más de los efectos nocivos que produce el consumo de grasas malas", dice el experto.

Se cree que las nueces contienen arginina, un aminoácido que el organismo utiliza para producir óxido nítrico.

Las nueces también contienen antioxidantes y ácido alfalinoléico, un aceite vegetal omega-3 con varias propiedades.

Sin embargo, los expertos advierten que no debe asumirse que se puede comer lo que se desee siempre y cuando se acompañe con nueces.

"Lo que aconsejamos es añadir nueces a una dieta saludable en la que están limitadas grasas saturadas", afirma el investigador.
